In my experience, the best way to evaluate recurrent MRSA infections is to check the nasal cultures of the patient and caregivers. Sometimes cutaneous carriage of the organism also needs to be checked and treated. Good hygiene with regular hand washing or alcohol sanitizers is essential. Treatment w...

How would you approach a patient with malignant spindle cell neoplasm of the scalp excised with a focally positive deep margin?

Although I agree that a re-excision would be helpful for margin clearance, it is probably unlikely that more tissue (which may be negative for Ca) would help delineate the pathology. An immunosuppressed transplant patient most likely has a cutaneous SCCa (with spindle cell features) and our surgeons...

I usually recommend treating these with Mohs surgery. Not every patient wants to have surgery for a benign tumor but these do tend to be in cosmetically sensitive areas and to slowly grow over time.

There are many effective medications for PsA out there now that don't have the risk of making their skin disease worse as is the case with HCQ. Patients are also less compliant with daily pills. To me, 1 in 10 is an unacceptably high number to flare their skin. When viewing through a historical lens,...

This is an exceptionally important question for clinicians. I'm a little biased as I run an autoimmune Myositis Clinic, but here are my two cents: Paraneoplastic dermatomyositis (DM) is a fairly common occurrence (roughly about 15% of all DM cases, but up to 30-40% in some subtypes, such as adult pa...

Cutaneous squamous cell carcinoma (CSCC) is primarily a UV radiation driven malignancy and harbors many mutations. These cancers generally respond very favorably to PD-1 pathway inhibition, and thus biomarkers do not currently play a role in patient selection for neoadjuvant cemiplimab in CSCC. In t...

This is a difficult question because the term "rashes" is extremely broad! I think that as in most of medicine, moving through the appropriate steps is critical. There can be a rush to move towards treatment, but remember, there is an order to medicine: History -> Physical -> Diagnosis -> Treatment ...

Hello, @Dr. First Last! If surgically resectable, then surgery. If a response to hedgehog inhibitors will reduce the morbidity of the surgery, then I would give one as tolerated (good responses seen within 3 months). Fosko et al., PMID 31836564Kahana et al., PMID 33988881

Prevention: Cleanse and moisturize the face daily using products that are appropriate for your skin type. If using a cloth mask, make sure it is washed regularly. Take off the mask when safe and appropriate. Skip makeup when wearing a mask. If makeup cannot be avoided, make sure it is labeled "non...

Do you use apremilast in combination with biologic DMARDs for psoriatic arthritis and/or psoriasis?

I have used apremilast in combination with a biologic in my practice. Monotherapy with biologics is not always effective in relevant treatment domains in patients with psoriasis and psoriatic arthritis. Then the choices are to switch to a different biologic agent, add a traditional DMARD or consider...
